Modification Service Form
If you purchased an item from us that requires you to send your pedal to us (such as a modification service), click on the link below. This will take you to a form that you can print out and send in with your pedal. The form will have all the information you need to send the pedal to us. If you don't have printing capabilities, please just write the information down on a piece of paper and send that in with your item(s).
